What Is DMEK?

DMEK is a highly advanced corneal transplant procedure that replaces only the single diseased cell layer of the cornea, the endothelium, and its thin supporting membrane (Descemet’s membrane). This layer is responsible for pumping excess fluid out of the cornea. When the endothelium stops functioning properly, as in Fuchs’ dystrophy or chronic corneal swelling, vision becomes cloudy, hazy, or distorted.

With DMEK, surgeons remove the damaged cells and replace them with an ultrathin sheet of donor tissue, just 10–15 microns thick, about the thickness of plastic wrap. Because the transplant is so anatomically precise, it delivers:

How DMEK Works Step-by-Step

Considering corneal surgery with DMEK? Let us walk you through it.

Step 1: Pre-Surgical Evaluation

Your surgeon will perform a comprehensive eye exam to determine whether DMEK is right for you. This includes measuring corneal thickness, assessing endothelial function, and reviewing your overall eye health.

Step 2: Removing the Damaged Tissue

During surgery, a small incision is made at the edge of the cornea. The diseased Descemet’s membrane and endothelium are gently removed, leaving the rest of the corneal structure intact.

Step 3: Preparing the Donor Graft

Donor tissue is ultra-thin and extremely delicate. It is carefully rolled and inserted into the eye through the microincision.

Step 4: Unfolding and Positioning the Graft

Once inside the eye, the donor tissue is unrolled and positioned against the back surface of the cornea. Most of the time, the graft unfolds naturally with careful manipulation of balanced saline and air.

Step 5: Air Bubble Support

A small air bubble is created to press the donor tissue into place, allowing natural adhesion to occur to the back of the cornea over the following hours.

Step 6: Closing and Recovery

The incision typically seals on its own. A protective shield is placed over the eye, and you rest in a face-up position for some time to help the graft attach securely.
