Rediscover Clarity and Comfort

At Eye Surgeons of Indiana, our corneal specialists combine advanced surgical techniques with compassionate, personalized care. DSAEK (Descemet’s Stripping Automated Endothelial Keratoplasty) is specifically designed for patients with endothelial corneal disease, such as Fuchs’ dystrophy or corneal swelling, providing a targeted solution that restores vision while minimizing recovery time and surgical risk.

What Is DSAEK?

DSAEK is a type of endothelial keratoplasty, a procedure that replaces only the inner layer of the cornea, the endothelium, along with its supporting Descemet’s membrane. The endothelium maintains corneal clarity by pumping excess fluid out of the tissue. When these cells fail, fluid accumulates, leading to cloudy or distorted vision, swelling, and discomfort.

Unlike full-thickness corneal transplants, DSAEK preserves most of your healthy cornea. By replacing only the diseased inner layers and a small number of corneal cells, the surgery reduces complications, protects your corneal structure, and allows faster visual recovery.

Why Choose Eye Surgeons of Indiana?

Our Indianapolis corneal surgeons are board-certified experts in advanced corneal procedures, including DSAEK. We prioritize:

Patient understanding and comfort

Clear explanations of each step and answers to all your questions.

Minimally invasive techniques

Small incisions and targeted tissue replacement for faster recovery.

State-of-the-art technology

Microsurgical instruments and operating microscopes ensure precise tissue placement.

Long-term vision management

Ongoing monitoring to detect early signs of rejection or complications.

Personalized care

Tailored surgical plans and detailed postoperative instructions for optimal results.

Recovery After DSAEK

While recovery is faster than full-thickness corneal transplants, following your surgeon’s instructions is essential for success:

  • Positioning: You may need to lie on your back for several hours post-surgery to help the tissue adhere.
  • Eye drops: Steroids and antibiotics reduce inflammation and prevent infection.
  • Activity restrictions: Avoid rubbing your eyes, heavy lifting, or vigorous exercise for several weeks.
  • Follow-up visits: Regular check-ups ensure the graft remains healthy and detect any early signs of complications.

Most patients notice improved vision within weeks, though full stabilization can take a few months.

Preparing for DSAEK Surgery

Before your procedure, a thorough evaluation ensures your eyes are healthy and ready for surgery. This includes:

  • Measuring corneal thickness and mapping corneal structure
  • Assessing overall eye health
  • Discussing your visual goals and expectations

Your surgeon will walk you through the procedure, recovery timeline, and what you can expect in the weeks following surgery.

How DSAEK Works

1. Removing the Diseased Layer

DSAEK begins with the careful removal of the damaged endothelium and Descemet’s membrane through a small incision at the edge of the cornea. Unlike traditional full-thickness transplants, only the inner layers are replaced, leaving most of your natural cornea intact.

2. Preparing Donor Tissue

Healthy donor tissue is meticulously prepared using automated microkeratome technology, which ensures the exact thickness needed for optimal function. The graft includes vital endothelial cells and Descemet’s membrane to restore corneal hydration and clarity.

3. Implanting the Donor Tissue

The donor graft is inserted through the small incision and gently positioned against the back surface of your cornea. An air bubble holds the tissue in place while it naturally adheres over the next one to five days, acting as a temporary support.

4. Securing and Protecting the Eye

The incision is typically self-sealing or closed with a tiny suture. A protective shield may be placed over the eye during early healing to prevent accidental injury.

Benefits of DSAEK

DSAEK is highly effective for patients with endothelial dysfunction, offering:

  • Clearer, sharper vision and improved contrast compared to before surgery
  • Faster recovery than full-thickness transplants
  • Reduced risk of rejection compared to penetrating keratoplasty
  • Preservation of corneal structure, supporting long-term eye health

Am I a Candidate for DSAEK?

You may be considered if you have:

  • Fuchs’ endothelial dystrophy or other endothelial disorders
  • Corneal swelling or clouding affecting vision
  • Pain or discomfort from corneal edema
  • No active eye infections or severe surface disease

Your Eye Surgeons of Indiana team will evaluate your cornea, overall eye health, and medical history to determine whether DSAEK is the right option for you.

Importance of Continued Care

Although DSAEK has high success rates, long-term follow-up is crucial. Donor tissue can occasionally fail or be rejected even years after surgery.

Routine eye exams allow our surgeons to detect issues early and intervene promptly to preserve vision and comfort.

DSAEK Frequently Asked Questions

How long does it take to recover from DSAEK?

Improved vision is usually noticed within a few weeks, but full stabilization may take several months.

Is DSAEK painful?

Surgery is performed under eye drop anesthesia with additional sedation administered by vein. Most patients experience only mild discomfort or a foreign-body sensation after the procedure.

Can my body reject the donor tissue?

While rare, rejection is possible. Early detection through follow-up visits ensures timely treatment and preserves vision.

How long does the donor tissue last?

With proper care, DSAEK grafts can last many years. Ongoing monitoring ensures continued corneal health and visual clarity.
